Read it like a story:

He looks like Robert De Niro; she's his Brigitte Bardot, and that's enough to hang around with those who think they know. But if you've got the image and symbols of success, you must be doing something right to keep up with the Jones's.

What's it mean?

Fame and fortune aren't everything. It requires sacrifices. While climbing to the top, you can't take time for a vacation. Once you've "arrived," appearances matter enough that plastic surgery might be required to stay at the top. Everybody wants some of your money.
